Abstract

The title of this paper allows wide latitude of discussion, and shall, therefore, not be limited to any particular phase but rather to its broader and more general aspect; and yet, not wishing to sacrifice the concrete for the abstract, the writings and experiences of others shall be used freely so that this treatise may prove not only interesting but that it may also serve to emphasize the importance of good surgical judgement.
