Abstract
A discharge plan was developed to assist nurses in the instruction of parents in the care of high risk infants. Guidelines were established and the plan was designed for use by a staff nurse in a hospital setting on units which accommodate high risk infants. A review of the literature included the need for discharge planning, the discharge period for parents of high risk infants as a time of crisis and the identification of the special needs of parents of high risk infants upon discharge. Based upon the review of the literature, a nursing plan for discharge teaching was formulated in relation to feeding, crying, skin care and temperature taking. The plan is designed for implementation at birth and to be used continuously until discharge.
