Abstract
The purpose of this study was to determine the effect of providing hospital nurses with guidelines and procedures for using a home care referral/transfer form. The referral form information written by the nurses was evaluated before and after they reviewed a self-learning packet containing the guidelines and procedures. An evaluation tool developed by the researcher was utilized for data collection. Frequency distribution, weighted means and t-tests were used to analyze the data. Referral form information provided by the nurses improved in the categories of patient understanding and attitude and living arrangements. A secondary partial analysis showed improvement in the categories of behavior and level of consciousness, treatments. Interventions from other disciplines, and correspondence of nurse information to physician information and patient diagnosis. The time lag between the patient's discharge date and the receipt of the referral at the home care agency decreased by 2.34 days. Additional inservice efforts were recommended to further improve the adequacy of the referral.
