Abstract
A nursing history was developed for use with children admitted to the hospital between the ages of eight and twelve years. It was designed for nurses to elicit information directly from the child. A review of literature describes the cognitive capabilities, psychosocial characteristics, and concepts of health and illness of children within this age group. This was used as a basis for the development of the questions in the nursing history. The nursing history is divided into the following three areas personal, illness, and health profiles. It takes approximately twenty to thirty minutes to complete and is presented along with guidelines for use.
