Abstract
A child health behavior assessment tool was developed for use with nine to twelve year old children. The literature supports the readiness of children of that age to learn and accept responsibility for personal health practices. The tool contains questions regarding five pertinent areas of childrens' health practices: safety, nutrition, exercise, stress and personal health maintenance behaviors. The tool is scored and written feedback given for every item in the questionnaire. Field testing was done with a 4-H group to establish the understandability of the language used. Validity and reliability studies are needed before the tool can be used for research purposes. It is recommended that further development of research-based health education materials be developed for children.
