Abstract
Chronic heart failure (CHF) is a major public health issue in the United States. With approximately 5.7 million persons living with CHF and 670 000 new diagnoses each year, CHF has now reached epidemic proportions (Hunt, 2005; Lloyd-Jones et al., 2009; McCullough et al., 2002; Schroeder et al., 2005). The estimated total cost of CHF in 2009 is estimated to be over $37 billion (LloydJones et al., 2009). Between 1994 and 2004, deaths from CHF increased 28%, and one in five persons diagnosed with CHF dies within one year of diagnosis (Schroeder et al., 2005)...
