Abstract
The intention of this study is to conduct a survey among high school teachers to discover whether there is a need for a teacher's record book that will provide for the recording of mental tests and pupil data and whether the effectiveness of the teacher's record book might be increased. This problem has as its second purpose the designing of a teacher's record book that might be workable and practical for recording mental tests and other pupil data for all students receiving instruction and guidance. Various types of teacher's record books have been published to record absences and marks in subjects, but the teachers have been left to their own resourcefulness to add other pupil data that was felt necessary to be included in the teacher's record book. It would seem that these teacher's record books, in order to be effective, should be designed to include space for mental tests and necessary pupil data. It is hoped that this record book might prove flexible for the particular teacher in any school system.
